Swansea University nation-wide study into the health, well-being and physical activity levels of our youth.
Thu 28 Nov 2024Dear parent,
I would like to invite your child to participate in a study on children’s well-being, mental health and physical activity levels. Your child may have taken part in a previous version of this study which was funded by the Welsh Government to monitor the effect of COVID-19 on children’s health and well-being. In recognition of the important insights provided, this has now become an annual physical activity and health monitoring initiative. If you agree, your child will be asked to complete an online questionnaire (should take 10-15 minutes) and some children will be selected to wear an activity monitor (like a FitBit) for a week.
Please click on this link to read more about the survey. If you are happy for your child(ren) to participate, please complete the parental consent form which you will find in the link below.
If you have any questions, please don’t hesitate to contact us.

Annwyl riant,
Hoffwn wahodd eich plentyn i gymryd rhan mewn astudiaeth ar les, iechyd meddwl a lefelau gweithgarwch corfforol plant. Mae'n bosib bod eich ysgol wedi cymryd rhan mewn fersiwn flaenorol o'r astudiaeth hon, a gafodd ei hariannu gan Lywodraeth Cymru, er mwyn monitro effaith COVID-19 ar iechyd a lles plant. I gydnabod yr wybodaeth bwysig a ddarparwyd, penderfynwyd cynnal y fenter hon yn flynyddol i fonitro gweithgarwch corfforol ac iechyd.I gydnabod y ddealltwriaeth bwysig a ddarparwyd, penderfynwyd cynnal y fenter hon yn flynyddol i fonitro gweithgarwch corfforol ac iechyd.
Os ydych yn hapus i'ch plentyn/plant gymryd rhan, cwblhewch y ffurflen ganiatâd i rieni sydd ar gael drwy'r ddolen isod.
Os oes gennych gwestiynau, mae croeso i chi gysylltu â mi.
